Supervisors’ Supervisory Preference Form

  • Please indicate your preferred time slots by selecting your 1st choice and 2nd choice. As you know, scheduling is an awesome task and providing an alternative time is essential. Supervision usually takes place at the University City Office. If you would like to supervise in a satellite office, please indicate your interest on this form. If requesting a satellite location, please make sure that at least one time slot is listed at the University City Office.
